Obituary of Paula M. Forte-Chambers

Paula M. Forte-Chambers, 48, of Zion, IL passed December 19, 2016 at Lake Forest Hospital with her family by her side. Paula was born March 25, 1968 in Chicago, IL. Survivors include her husband, Tom Chambers; children, Courtney, Matthew, Stephanie, Melissa, Jessica, Nicole, Christina, Eric, Emma, and Jason; 11 grandchildren, Aliyah, Julian, Mason, Aiden, JJ, Anthony, Sophia, Aaron, Jayden, Brandon, and Aria; siblings, Cliffton, Lisa, Sherry, Thomas, and Douglas; and a host of many close friends. She was preceded in death by her daughter, Samantha; parents, Clifford and Cecilia Forte; and brother, Darrell. Visitation will take place Wednesday, December 21, 2016 from 6:00 until 9:00 p.m. at Congdon Funeral Home, 3012 Sheridan Rd., Zion, IL. A memorial service will take place at 7:00 p.m. at the funeral home.
